FORM INSTRUCTIONS

1. Click on blue hyperlink

2. Click “Open”

3. A new window will open and form will appear

4. Click on “Enable Editing” (if you do not see EE button that means one of two things: 1) you may automatically edit or 2) you must print out form and fill it out by hand)

5. Type in answers

6. Save completed form with “Save As”

7. Name it whatever you want

8. Email form to Audrey by attaching it to email (audrey@ctumc.org)

Youth Department's Annual Minor Consent/Release Form

To be completed annually by parent/guardian
Required before Student can participate in events that are overnight or off campus
If Student has insurance, make copy of insurance card (front and back) and bring it with you the next time you’ll see Audrey so she can add it to Student’s file.
